La fruta de estrella, Averrhoa carambola o carambola pertenece a la familia de Oxalidaceae, se muestra en la figura 1. Es una fruta que se consume habitualmente en Asia y Latinoamérica, aunque con la globalización es fácil encontrarla en cualquier país del mundo. Se le han atribuido numerosas propiedades terapéuticas, sin embargo la carambola es una fruta rica en oxálico1–3. En los pacientes con enfermedad renal crónica el consumo de la fruta de estrella se relaciona con clínica neurológica que puede oscilar entre hipo, confusión, hasta coma y muerte2,4. A pesar de su consumo generalizado, los casos publicados de fracaso renal agudo (FRA) secundario al consumo carambola son una decena1,2,5–8. Presentamos un nuevo caso de FRA por consumo de carambola. Paciente mujer de 55 años, natural de Perú, con antecedentes personales de exfumadora, hipertensión arterial en tratamiento con medidas higiénico-dietéticas, diabetes mellitus tipo ii con buen control con 2 antidiabéticos orales y dislipemia en tratamiento con una estatina. Acudió al servicio de urgencias por dolor lumbar bilateral con náuseas, dolor abdominal y astenia. En la exploración física destacaba una presión arterial de 140/80 mmHg, una leve deshidratación mucocutánea, así como dolor a la palpación de ambas fosas renales con puño-percusión renal bilateral negativa. Los datos analíticos que presentaba eran hemoglobina 13,6 g/dl, leucocitos 8.300/ l, creatinina sérica 4,55 mg/dl, urea 113 mg/dl, sodio 143 mEq/l, potasio 5,1 mEq/l, pH 7,30, pCO2 37 mmHg y HCO3 18 mmol/l. En la orina destacaba: sodio 36 mEq/l, potasio 31 mEq/l, creatinina 101 mg/dl y proteinuria 20 mg/dl. En
